A while ago I had a patient come to see who had high blood pressure, for which she was already on medication from her GP. She had come to see me about an unrelated health problem. In the course of treatment I gave her some potassium chloride tablets for a separate health issue. Over the next few days her blood pressure went up significantly. I was a first a bit perplexed. Potassium is known to lower blood pressure so why had this caused her blood pressure to rise? Then it dawned on me that common salt ,sodium chloride, (which is known to raise blood pressure), and potassium chloride both have one component in common; namely chloride! I started to think perhaps it is the chloride and not the sodium that raises blood pressure in common salt. So I did some research and came across the follow paper …"Contributions of Sodium and Chloride to NaCl-Induced Hypertension by Theodore A. KotchenFrom the Department of Medicine, Medical College of Wisconsin, Milwaukee, Wis.The recommendation to avoid high dietary "salt” intake for the prevention and treatment of hypertension is often expressed in terms of dietary sodium. However, a consistent body of evidence suggests that the chloride component of salt is an important contributor to NaCl-induced elevations of blood pressure…”What if naturally occurring sodium doesn’t raise blood pressure significantly at all? What if it is only added salt i.e. sodium chloride that those with high blood pressure should be concerned about. This also begs the question, should people be limiting foods high in natural sodium content which have no added salt? What is for certain though is that we shouldn’t be recommending salt substitutes containing potassium chloride for anyone with high blood pressure!
